Central Adelaide Local Health Network (CALHN) is seeking a Placement Officer at Hampstead Rehabilitation Centre, Northfield SA 5085. The role is permanent full time, ASO3 salary range $74,095 to $78,694 p.a.

plus superannuation and salary packaging. You will coordinate aged care applications, support discharge planning and liaise with patients, families and providers to find care options that meet individual needs and budgets.
