UnitingCare Health's BlueCare division seeks a Care Partner to deliver person-centred, high-quality community and aged care. You will oversee care planning, coordinate services with clinical teams, and support clients in navigating funding and resources.

Based in Bowen, Proserpine and surrounding areas, you’ll travel between locations Monday to Friday 8am-4pm, providing on-site and in-community care with a supportive team and a solid Not-for-Profit culture.
