3Bridges Community Limited is seeking an experienced Care Partner to join our Aged Care team in Sydney, on a full time fixed-term 18-month parental leave contract at Carss Park. You will manage caseloads, develop care plans and coordinate services to support clients’ independence, safety and wellbeing.

You will collaborate with frontline staff and internal teams, ensuring person-centred, responsive care aligns with funding requirements and quality standards.
