Headquartered in Broome, Western Australia, BRAMS provides holistic, culturally protected primary healthcare, aged care, and community programs to Aboriginal people throughout the Kimberley region.

Our vision: Healthy People – Strong Community – Bright Future.

About the Role

BRAMS is seeking an Aged Care Registered Nurse to deliver clinical care and care coordination for older people receiving Support at Home, CHSP and Transition Care Program services. Working closely with consumers, families and service providers, you'll help older people remain safe, independent and connected in their homes through person-centred care planning, clinical support and service coordination. Reporting to the Aged Care Clinical Lead, you will play a key role in ensuring high-quality, culturally safe care that meets consumer goals and program requirements.

Your day-to-day will include:

- Conduct comprehensive clinical assessments and ongoing health reviews.
- Deliver nursing care including wound management, chronic disease monitoring, medication support, and health education.
- Develop, implement, and monitor care plans and risk management strategies.
- Monitor consumer wellbeing, respond to changing needs, and escalate concerns appropriately.
- Support consumers and carers with health education and self-management strategies.
- Provide culturally safe, trauma-informed care to Aboriginal consumers and their families.
- Work collaboratively with support workers and multidisciplinary teams to achieve positive outcomes.
- Maintain accurate, timely, and compliant clinical documentation.

About You

You are a compassionate and experienced Registered Nurse with strong clinical assessment and care coordination skills. You are committed to supporting older people to live safely and independently at home, work collaboratively with multidisciplinary teams, and deliver culturally protected, person-centred care.

You communicate effectively, maintain high professional standards, and are dedicated to achieving positive outcomes for Aboriginal and Torres Strait Islander consumers and their families.

You’ll bring:

- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), with no conditions that would restrict practice in aged care or community settings.
- Demonstrated experience delivering clinical nursing care to older people in community, aged care, or post-acute settings, including assessment, monitoring, and management of complex health needs.
- Demonstrated understanding of, or experience working within, the Support at Home Program and/or Transition Care Programme, including care coordination requirements and consumer-centred service delivery.
- Proven ability to undertake comprehensive clinical assessments and contribute to the development, implementation, review, and adjustment of care plans that reflect consumer goals, risks, and changing needs.
- Experience coordinating services for older people living at home, including working with multidisciplinary teams, families, carers, and external providers to support safe and timely service delivery.
- Demonstrated ability to identify clinical and service delivery risks, respond appropriately, and escalate concerns in accordance with clinical governance and organisational procedures.
- High level of competence in clinical documentation and record keeping, with the ability to maintain accurate, timely, and compliant records to support quality, reporting, and audit requirements.
- Strong interpersonal and communication skills, with the ability to engage effectively with consumers, families, carers, colleagues, and external stakeholders.
- Demonstrated commitment to culturally safe, respectful, and trauma-informed practice, particularly when working with Aboriginal and Torres Strait Islander consumers and their families.
- High level of integrity, accountability, and commitment to confidentiality,

ethical practice, and BRAMS values.

Highly regarded:

- Experience working within an Aboriginal Community Controlled Health Service (ACCHO), or in a remote or regional health setting.
- Knowledge of aged care quality standards, Support at Home program guidelines, and clinical governance requirements.
- Experience using electronic clinical, care coordination, or quality systems (e.g. MMEx, Sandwai, LOGIQC, or similar).
- Postgraduate qualifications or additional training in gerontology, chronic disease management, dementia care, wound care, or community nursing.
- Experience supporting service transitions, including hospital discharge and post-acute care coordination.
