Grampians Health is seeking a Memory Support Nurse in Stawell for a fixed-term part-time role (0.4 FTE). The nurse will deliver specialist dementia care, coordinate care plans, and lead dementia initiatives in a community services setting.

The role requires current AHPRA registration, solid dementia knowledge, and experience in aged care or community health. Flexible arrangements and salary packaging are offered with close date 15 November 2026.
