Uniting Age Well is seeking a dedicated Care Advisor (Case Manager) for the Barwon Home Care Program in Geelong. You will manage your own client caseload, promote independence, and support clients to live confidently at home. The role is based in Belmont, moving to Waurn Ponds in December, with a 9am–5pm Monday–Friday schedule.
You will oversee initial assessments, care planning, budget management, and coordination with a supportive team to deliver high-quality aged care services.
