SASScare are seeking an experienced Disability Support Worker in Elwood, the South East Suburb of Melbourne, to join our support team.

We are seeking a female support worker - as requested by the participant - for the following days and times:

- Tuesdays 12 - 3pm
- Thursdays 12 - 3pm
- Fridays 12 - 3pm
- Every second Saturday 12 - 3pm

You'll provide care to a participant in their home and local community, supporting their independence and improving their quality of life through activities that include:

- Daily living assistance, help at meal times and general mobility
- Providing care in accordance with their individualised Care Plan
- Supporting with community based recreational and social activities
- Transportation and assistance with shopping and appointments

What you'll need:

- Cert III Individual Support (Disability) or Cert IV in Disability
- NDIS Worker Screening Check Clearance
- Current National Police Check
- Drivers License, your own vehicle and full Comprehensive Insurance
- First Aid (HLTAID011) & Cardiopulmonary Resuscitation (CPR) (HLTAID009)
- Current Working with Children Check
