About the Role
We are seeking an experienced Case Manager to join a growing Support at Home team in Adelaide.
This is a rewarding role where you'll partner with older Australians and their families to coordinate tailored support services that promote independence, wellbeing and quality of life.
Responsibilities
Manage a portfolio of aged care clients




Complete assessments and develop care plans
Coordinate support services and providers
Monitor budgets and funding allocations
Conduct regular reviews and reassessments
Maintain compliance and documentation
Ideal Candidate
Previous Case Management or Care Coordination experience
Experience within Home Care Packages or Community Care
Strong knowledge of aged care legislation
Excellent communication and stakeholder engagement skills
Current Driver's Licence
