Raise awareness of health promotion in PCN Practices9.
Support PCN Leads with uptake of national screening programmes10.
Support PCN Leads with immunisation programmes11.
Support delivery of the PCN covid vaccination programme12.
Direct liaison with multi agencies to coordinate care for patients13.
Lead PCN patient participation group14.
Run PCN Extended access audits15.
To support patient/carer contact roles, and collate patient and carer feedback on their experiences16.
Support Quality and Outcome Frameworks and other DES/LES specifications17.
Maintain and develop PCN engagement with all practice staff and encourage best practice18.
Perform any other duties assigned by management
