For over 15 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.Our friendly, compassionate and experienced team makes this possible.We are currently looking for dedicated Support Worker to join a team near the Wangaratta area to work with a customer that requires full time care.The roster for this team is broken up into 3 time slotsMorning - 8am to 2pmAfternoon - 2pm to10pmSleepover - 8pm to 10amPay rates range fromWeekdays - $***** - $***** per hourWeeknight - ***** - $***** per hourWeekend and Public holiday rates - $*****+ per hourDuties and responsibilities include:Personal care: showering, dressing, grooming and medication assistanceSocial support/transportation for social activities, shopping, travelling to medical appointmentsDomestic assistance:



light household cleaning, meal preparationSupporting the customer in the use of aids and personal equipmentSupporting the customer with complex and/or challenging needsMonitoring the well-being of the customer and reporting on their progressProvide updates on the customer's progress following each serviceAbout you:Experience in the field is an advantageGood interpersonal skills & written/verbal communication skillsAbility to build rapport & positive working relationshipsComfortable using technology (i.e. mobile app) for accessing rosters & recording customer notes/feedbackWhat you will need:NDIS Worker Screening Clearance / Criminal Record CheckCurrent First Aid certificate/CPRCurrent Australian Drivers LicenseReliable, registered, and insured vehicleSmart phone with internet accessCertificate III or IV in Aged Care, Home & Community Care, Individual Support or Disability ServicesMedication Administration certificate is an advantage
