About the opportunity

Direct Care Australia is seeking an experienced and highly organised Care Partner to join our Community Aged Care team on the Gold Coast.

This is an ongoing opportunity for someone with demonstrated experience in Home Care Packages, Support at Home or community aged care case management who enjoys working directly with older people while taking ownership of their care coordination responsibilities.

You will manage an allocated portfolio of participants, undertaking assessments, developing individualised care plans, coordinating services and monitoring changing needs to help older people remain safe, independent and connected in their communities.

We are seeking someone who combines genuine compassion with sound professional judgement, initiative, strong documentation skills and the ability to manage competing priorities.

What you'll be doing

- Care management: Independently manage a portfolio of Support at Home participants, maintaining regular engagement and oversight of their care arrangements.

- Assessments and reviews: Conduct participant intakes, assessments and scheduled reviews, including face-to-face visits across the Gold Coast and surrounding regions.

- Care planning: Develop and maintain person-centred care plans that reflect participants' assessed needs, goals, preferences and identified risks.

- Service coordination: Arrange and monitor services, communicate with scheduling teams and frontline workers, and coordinate nursing, allied health and other specialist referrals where required.

- Budgets and funding: Monitor participant budgets, service utilisation and changing support requirements, ensuring services remain appropriate and within available funding.

- Quality and documentation: Maintain accurate, timely case notes, assessments, care plans and service records, identifying and escalating clinical, safeguarding and service delivery concerns.

- Participant engagement and growth: Build trusted relationships with participants and families, respond promptly to new referrals and contribute to participant satisfaction, retention and appropriate service growth.

About you

You are an experienced community aged care professional who understands that effective care management requires both strong participant relationships and disciplined follow-through.

We are particularly interested in candidates who offer:

- Demonstrated experience in Home Care Package or Support at Home case management, care coordination or a closely comparable community aged care role.

- Practical experience undertaking participant assessments, care planning, reviews and service coordination.

- Strong organisational skills, with the ability to independently manage a caseload and competing priorities.

- Sound understanding of participant funding, budgets and contemporary aged care requirements.

- Excellent documentation skills and confidence using electronic client management systems.

- The ability to recognise changing needs, identify risks and coordinate appropriate clinical or specialist support.

- Strong communication skills and a cooperative, professional approach to working with participants, families and colleagues.

Relevant qualifications in nursing, community services, aged care, social work or allied health will be highly regarded. Substantial directly relevant experience will also be considered.

You will need to hold a current Australian driver's licence, meet applicable aged care worker screening requirements and be willing to travel across the Gold Coast, Brisbane and Sunshine Coast regions for participant visits. A company vehicle is provided for work-related travel.

This is a Care Partner / case management position, not a frontline personal care or support worker role.

Why join Direct Care Australia?

Direct Care Australia is an established Australian provider delivering community aged care, disability and related support services across South Australia and Queensland.

We are continuing to develop our Support at Home services, with an emphasis on person-centred care, professional accountability, quality and responsive service delivery.

We offer:

- A full-time, ongoing position based at our Southport office.

- Competitive remuneration aligned to SCHADS Award Level 4, commensurate with qualifications and experience.

- Access to a company vehicle for work-related participant visits.

- Access to organisational support and established referral pathways for nursing, allied health and specialist clinical services.

- The opportunity to contribute to a growing Community Aged Care service.

- Meaningful, relationship-focused work supporting older people to live independently in their own homes.

Interested in joining us?

If you have experience in community aged care case management and are looking for an opportunity to make a genuine difference, we would welcome your application.

Please submit your resume and a brief cover letter outlining your relevant Home Care Package, Support at Home or community aged care experience.

We are seeking to appoint as soon as practicable. Applications will be reviewed as received, and suitable candidates may be contacted for interview before the closing date.
