Better Health Network in Melbourne is seeking an experienced Young Person's Case Manager to deliver intensive, trauma-informed support to 15–25-year-olds experiencing or at risk of homelessness in Port Phillip and surrounding areas.

You will work within RhED, providing outreach, assessments, planning, and advocacy across housing, health, legal and education services, contributing to safer, more secure futures for young people.
