Care Partner / Care Coordinator - Support at Home

McArthur is seeking an experienced Care Partner or Care Coordinator to join a leading community aged care provider in Ashfield for an immediate start on a six-week temporary assignment.

This is a fantastic opportunity for an experienced community aged care qualified who understands the Support at Home programme and enjoys working directly with older Australians to ensure they receive the services and support they need to remain independent at home.

We are particularly interested in candidates with previous experience managing Home Care Packages or Support at Home clients who can confidently navigate care plans, individualised budgets, funding guidelines and service coordination.

About the Role

Working within an established Care Coordination team supporting approximately 150-190 clients, you will assist with the day-to-day management of client needs, care planning, funding and service delivery.

Your responsibilities will include:

- Conducting monthly wellbeing calls with clients to monitor their needs, satisfaction and ongoing support requirements.

- Reviewing individual client budgets and monitoring expenditure against available Support at Home funding.

- Reviewing and updating care plans to reflect changing client needs and circumstances.

- Coordinating home care services and following up with clients, families and external service providers.

- Assisting clients with Support Plan Reviews and applications for additional funding where required.

- Providing guidance to clients and Care Coordinators regarding Support at Home funding, including programme inclusions and exclusions.

- Maintaining accurate client records, documentation and case notes.

- Working collaboratively with Care Partners,



Care Coordinators and service delivery teams to ensure timely, person-centred support.

About You

We are looking for someone with hands-on community aged care experience who can quickly become a valuable member of the team.

Ideally, you will bring:

- Previous experience as a Care Partner, Care Coordinator, Case Manager or similar within community aged care.

- A strong working knowledge of the Support at Home programme, including funding arrangements, service inclusions and exclusions.

- Experience reviewing care plans, monitoring client budgets and coordinating services.

- An understanding of Support Plan Reviews, reassessments and additional funding processes.

- Excellent communication skills and confidence engaging with older Australians, their families and service providers.

- Strong organisational skills, attention to detail and the ability to manage competing priorities.

- Confidence using client management systems and maintaining accurate documentation.

- The ability to work independently and contribute effectively within an established team.

Previous experience working with Home Care Packages and the transition to Support at Home will be highly regarded.

As this is a short-term assignment, we are seeking someone who can commence immediately and contribute with minimal training.
