Nursing Community Care Manager (Senior RN Management role)The "Care Designer" - Nursing Community Care Manager (Senior role RN Grade 4) - will manage a team of Nurses and Caregivers who are passionate about making a difference in aged and disability community care. You will lean on and learn with a Leadership team who care about and admire each other, making a real difference in the aged and disability community care sector.Key responsibilitiesSchedule and coordinate care visits, ensuring optimal allocation of resources and timely delivery of services to clientsAct as the primary central point of contact between clients, families, healthcare professionals, and care staffConduct initial client assessments and care planning meetings to identify individual care needs and preferencesManage client records, documentation, and care plans in accordance with aged care regulations and organizational policiesMonitor care quality and client satisfaction, address concerns promptly, and **escalate** issues as necessaryProvide administrative support to ensure smooth operations while adhering to privacy legislation, duty of care, and confidentiality requirementsAbout youNursing qualification (Registered Nurse) ideally with experience in both aged care and disability settingsPalliative Care experience highly desirableProven experience in care coordination, case management, or a similar role within healthcare or aged careSound knowledge of aged care regulations, standards, and best practice frameworksEssential you have proficiency in electronic health records and computer-based scheduling softwareStrong organisational, time management, communication, and problem-solving skills paired with sound clinical judgmentWill require current First Aid and CPR certificationWorking with Children Check (WWCC) and Working with Vulnerable Persons screeningCompetitive remuneration reflecting experience and qualificationsFlexible working arrangements where possible to support work-life balanceOngoing career development,
