Job Description

Kindness in Action at IRT Kindness in Action is how we bring our purpose to life. It guides how we work, how we treat one another, and how we support our customers, families and communities. - Administer and monitor medications aligned to nursing registration and in accordance with authorised orders, relevant legislation, professional standards and organisational policies.

Identify and report medication errors, omissions and adverse reactions as per registration obligations and IRT policies and procedures.

- Provide and coordinate nursing interventions, including wound care, pressure injury prevention, pain management, continence care, skin integrity management and chronic disease monitoring.
- Support residents requiring dementia care, palliative care, end-of-life care and management of complex health conditions in accordance with their assessed needs and preferences.
- Recognise and respond to emergencies, acute illness and changes in residents' conditions, initiating appropriate clinical interventions and contacting medical practitioners or emergency services when required.
- Provide clinical leadership, supervision and support to Enrolled Nurses and care staff.

Delegate tasks appropriately, considering staff competence, resident risk and scope of practice, and monitor the quality and safety of delegated care.

- Coordinate case conferences, medical reviews, referrals and multidisciplinary collaboration to promote continuity of care and positive resident outcomes.
- Coordinate GP and health professional reviews, referrals and multidisciplinary care, ensuring recommendations are followed up.
- Review and manage clinical incidents and infections, including assessment, immediate actions, follow-up, escalation and implementation of risk controls.
- Competently use organisational electronic systems for clinical care, medication management, incident and infection management, education and other required clinical activities.
- Maintain accurate, timely and confidential clinical documentation, including assessments, progress notes, care plans, medication records, clinical observations, handovers and incident reports.
- Implement and monitor infection prevention and control measures, including hand hygiene, standard and transmission-based precautions, infection surveillance and outbreak management.
- Assess and manage clinical risks, including falls, pressure injuries, malnutrition, dehydration, choking and medication-related harm.
