Western Health is seeking a Clinical Nurse Consultant (CNC) or Allied Health Senior Clinician to join the SNAP service in Melbourne. The role focuses on high-level assessments, rapid-response planning, and guiding multidisciplinary teams to optimise patient pathways from acute to subacute care.

The successful candidate will have AHPRA registration, a postgraduate nursing qualification, and a commitment to Western Health values.
