About the Role

Comfort Home Care is seeking a compassionate, reliable and motivated Support Worker to assist a client with community participation in Fawkner, Victoria.

The successful candidate will support the client to access the community, participate in social activities, attend appointments and engage in activities that promote independence, confidence and overall wellbeing.

We are looking for someone who is patient, respectful, dependable and passionate about helping people achieve their individual goals.

A valid driver's licence is essential for this position.

Key Responsibilities

- Support the client to participate in community and social activities.
- Assist with attending appointments, shopping and community outings.
- Encourage independence, confidence and social engagement.
- Provide companionship and emotional support.
- Support the client to achieve their individual goals and participate in activities of their choice.
- Follow the client's support plan and respect their preferences and choices.
- Maintain appropriate skilled boundaries.
- Complete progress notes and required documentation following support shifts.
- Follow workplace health and safety procedures and relevant care requirements.

Essential Requirements

- Valid driver's licence.
- Reliable and responsible approach to work.
- Strong communication and interpersonal skills.




- Ability to work independently and follow individual support plans.
- A patient, respectful and person-centred approach.
- Willingness to travel within the local community as required.

Desirable Qualifications and Checks

- Certificate III in Individual Support, Disability, Aged Care or a related field.
- Previous experience in disability support or community participation.
- Current NDIS Worker Screening Check, where required.
- Current First Aid and CPR certificates.
- National Police Check, where required.
- Access to a reliable, appropriately insured vehicle if required for client transport.
