As Team Leader, you will lead a small team of support workers. You’ll deliver hands-on personal care and community support while making sure care is safe, consistent and person-centred. You will be the main link between participants, families, the support team and our clinical and management staff.

What you’ll do

- Lead, guide and mentor support workers on shift, including onboarding current staff and buddy shifts
- Provide personal care, daily living and community access support to female participants
- Support medication administration and health monitoring in line with care plans (Enrolled Nurses: within your scope of practice)
- Follow and help put into practice Behaviour Support Plans, risk assessments and care plans
- Check shift notes, incident reports and documentation for accuracy and completeness
- Report changes in participant health, behaviour or needs to the RN or management
- Help with rosters, team meetings and day-to-day coordination of the house or service
- Build positive relationships with participants, families, support coordinators and allied health

About you

- Support Worker applicants:



Certificate III or IV in Individual Support, Disability or Mental Health (or similar), plus at least [2] years’ disability support experience, including some time in a lead or senior role
- Enrolled Nurse applicants: current AHPRA registration as an Enrolled Nurse with no conditions, plus NDIS or disability experience
- Experience with complex needs, mental health or behaviour support is highly regarded
- Confident leader with strong communication, problem-solving and documentation skills
- Calm, patient and respectful, with a genuine passion for supporting people with disability

- NDIS Worker Screening Check
- Working With Children Check
- Current First Aid and CPR
- NDIS Worker Orientation Module certificate
- Full driver’s licence and a reliable, comprehensive-insured vehicle
- Flexibility to work a mix of shifts, which may include weekends and sleepovers

Pay: $38.00 – $45.00 per hour

Expected hours: 15.0 – 40.0 per week
