Our client is seeking a dedicated and compassionate Home Care Coordinator to join their team located in the Southwest to support the owners, care team and grow the client base. This is a fantastic opportunity for someone who enjoys building relationships, coordinating quality care, managing client services and being part of a supportive business with exciting growth opportunities. You will play an important role in coordinating and supporting approximately 40-60 clients across Bunbury, Busselton, Harvey and Donnybrook, with a particular focus on supporting the growth of our client base in the Busselton area.

Key Responsibilities

- Managing and supporting a client base of approximately 40-60 clients .
- Preparing, reviewing and monitoring individual care plans and client budgets.
- Coordinating services and arranging client schedules.
- Liaising with support workers and managing day-to-day service requirements.
- Maintaining regular contact with clients and their families.
- Conducting client visits and monitoring changing care needs.
- Supporting clients receiving Support at Home (SaH) services.
- Ensuring services are delivered in line with individual needs, budgets and care plans.
- Building strong relationships with clients, families and support workers.
- Identifying opportunities to grow and develop our client base, particularly throughout the Busselton area.
- Working closely with the owners to support the day-to-day operations and continued growth of Pearl Home Care.

About You

- Previous experience working as a Home Care Coordinator/Care Partner.




- Experience with Home Care Packages, CHSP and/or the Support at Home program.
- Experience in either a clinical or non-clinical care environment.
- Registered Nurse and current AHPRA - Desirable but not essential
- Understands care planning, budgets and service coordination.
- Has excellent organisational, communication and scheduling skills.
- Confident working with clients, families and support workers.
- Compassionate, approachable, proactive and solutions-focused.
- Can work independently while contributing positively to a team.
- Passionate about delivering quality and person-centred care.
- Current National Police Clearance and Drivers Licence
