Australian College of Nursing in Hervey Bay is seeking a Nurse Navigator to support integrated care at Hervey Bay and Maryborough Hospitals.
You will coordinate personalised, patient-centred care, connect patients with health services, and work with multidisciplinary teams to support recovery and wellbeing.
The role is a temporary full-time position (76 hours per fortnight) from appointment for up to 18 months, with option for temporary part time (38 hours p.f.) to 31 December ****; remuneration
