The Social Work Grade 3 Clinician is expected to work independently whilst at the same time contributing strongly to the multidisciplinary mental healthcare team, and to initiate and participate in teaching, training, research and quality activities.
Work under the Mental Health and Wellbeing Act **** to skilfully engage consumers, carers, families and supporters Co-ordinate, oversee and monitor discharge planning as part of a multidisciplinary team Provide crisis intervention for discharge plans where required Work within MARAM and Information Sharing framework Liaise with other St Vincent's Mental Health services and external agencies to ensure effective and efficient discharge is managed Liaise with community groups and relevant agencies to develop and review community networks which enhance continuity of care Supervise and provide training to social students and other non social work staff according to service policy, University Schools of Social Work and AASW requirements Your Contribution Approved degree in Social Work, and eligible for membership of AASW Minimum of 2 years post qualification in mental health (desirable)

The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincent's Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS).
Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention), Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services.
