In this role, you will coordinate and review services for older people receiving support at home (SAH), working closely with clients, families and providers to deliver high-quality,



person-centred care.Key responsibilities include:Conducting assessments and developing care plansCoordinating services under the Support at Home ProgramMonitoring and reviewing client outcomesMaintaining accurate records and compliance requirementsBuilding solid relationships with clients, families and service providersParticipating in an on-call rosterAbout YouYou will have:Experience in care coordination, case management or a similar roleA Diploma of Community Services, Nursing, Aged Care or related qualificationKnowledge of community aged care programs, including CHSP and Support at HomeStrong communication, organisational and administrative skillsA commitment to person-centred care and continuous improvementAppointment is conditional upon a satisfactory criminal record check and proof of influenza vaccination.
