Working within a case management model, you will provide advanced clinical assessment, care coordination and therapeutic intervention in a community setting.Key responsibilities include:Conducting comprehensive assessments using a range of formal and informal assessment tools.Developing, coordinating and reviewing tailored care plans that address complex client needs.Facilitating access to appropriate community, health and support services.Building effective partnerships with clients, families, carers and multidisciplinary service providers.Providing expert clinical advice and advocacy to optimise client outcomes.Leading outcome measurement and evaluation activities to inform evidence-based practice and continuous service improvement.Supporting the development of students and less experienced clinicians through education,
