You will collaborate with multidisciplinary teams to optimise patient flow and ensure secure, person-centred discharge planning across the continuum of care.

You will establish systems, act as a specialist resource, lead staff education, and monitor KPIs to drive quality improvement in a world-class public hospital setting.
