Sydney Local Health District is seeking a Social Worker to join the OOHC Health Pathway Program at Croydon Health Centre. The role centers on trauma-informed psychosocial assessment and Health Management Plans for children in Out of Home Care, aged 0–18, in collaboration with the OOHC Health Co-ordinator.
The position is temporary part time (24 hours/week) with potential extension. Must hold a Social Work degree eligible for AASW and a NSW Driver Licence; Tuesday work required.
