NSW Health in Cootamundra is seeking an Allied Health Assistant to join the Transitional Aged Care Program.
You will support discharged patients, assist with coordinated care plans, and work with a multidisciplinary team to help clients transition safely from hospital to the community.
The role offers part time hours up to 30 per week, a salary range of AUD 65,704 to AUD 74,286 per year (pro rata with super and 17.5% leave loading where applicable), and opportunities for professional development
