Pearl Home Care Gold Coast is seeking an experienced Care Partner to coordinate Support at Home packages and manage a caseload from onboarding through ongoing review. You will set up services, craft care plans, manage budgets, and serve as the primary contact for clients and families.

The role is office-based in Hope Island with hybrid, work-from-home days. You'll work with an in-house clinical team, not a head office, and enjoy prospects for growth within a locally owned provider.
