Western NSW Local Health District in Parkes, NSW, is seeking a Nurse Practitioner to lead the Hospital in the Home and Ambulatory Care Services for Lachlan Health Service (Parkes & Forbes). The HiTH NP will identify and refer suitable patients from ED, inpatient units, aged care facilities, and community nurses to HiTH to prevent admission to hospital or ED visits.
Role requires leading a team of clinicians, working across Parkes and Forbes sites, with shifts 08:*****:30, Monday to Friday.
