The role supports older adults through short-term rehabilitation and care coordination, helping regain independence and ensuring a protected transition home.

You will work with a multidisciplinary team to deliver compassionate, person-centred care, aligned with NSW Health CORE values and a solid focus on culturally respectful practice.
