NSW Health in Parkes and Forbes invites applications for a Nurse Practitioner–Hospital in the Home role. The HiTH NP leads the Lachlan Health Service HiTH team, working with ED, inpatient and community services to deliver patient-centred care and prevent unnecessary hospital presentations.

Role involves identifying eligible patients, coordinating referrals, and working across Parkes and Forbes sites. Shifts are 08:00–16:30, Monday to Friday, with permanent full time employment and a salary
