Senior Worker / Youth and Support Mentor – Leadership PathwayPacific Supports & Mentoring PTY LTD – Burleigh Heads QLDWe are seeking experienced Disability Support Workers and Youth Workers, including applicants capable of stepping into Senior Support Worker / Team Leader responsibilities.Depending on your experience and capability, responsibilities may include:Provide high-quality 1:1 disability support, youth work and mentoring, including participants with complex support needs and behavioursTake a proactive approach to participant goals, engagement, independence, life skills and community participationBuild strong relationships with participants while maintaining skilled boundaries, accountability and duty of careDemonstrate maturity, initiative and confidence when managing challenging situations and making decisions in the communityMaintain high standards across case notes, incident reporting, care plans, risk management and shift documentationCommunicate professionally with families, stakeholders, support coordinators and PSM managementIdentify changes in participant needs,



risks or service delivery concerns and escalate these appropriatelyExperienced applicants may also take on additional responsibilities including:Acting as a Senior Support Worker / Team Leader across allocated participants and support teamsProviding guidance, mentoring and on-shift leadership to other support workersSupporting management with participant engagement, staff performance and service qualityAssisting with client reviews, care planning, risk management and case-management-style oversightMaintaining strong communication with families, stakeholders and external service providersIdentifying gaps within a participant's support and working with management to improve service delivery and outcomesAssisting with the onboarding and integration of new workers into participant support teamsEscalating incidents, risks, behavioural concerns and operational issues to managementHelping maintain consistency and accountability across participant support teamsWe are particularly interested in applicants with backgrounds in Senior Support Work, Youth Work, Team Leadership, Case Management, Community Services, Mental Health or Residential Care who still enjoy being hands-on with clients.For strong performers, there is a genuine pathway into Team Leader, Service Coordination and broader management responsibilities as PSM continues to grow.
