About the role

This is a rewarding, purpose-driven role where no two days are the same. You'll deliver personalised, high-quality support that promotes independence, choice and control, community participation, and life skills development across home, work, social, and recreational settings. You'll support participants through a wide range of meaningful activities and work closely with families, carers, case managers, and allied health professionals.

Key responsibilities

- Assist with daily living and in-home support tasks

- Support employment, education, and community participation

- Provide one-on-one or small group support tailored to individual plans

- Support clients during outings, appointments, and community activities

- Deliver support aligned with NDIS goals and individual service plans

- Monitor wellbeing and report concerns proactively

- Complete clear, accurate documentation such as daily notes and progress reports

- Support behaviour support and skill-building programmes

- Act as a Key Worker where applicable, including monthly reporting and case reviews

- Maintain up-to-date client records, health plans, and behaviour support plans

About you





- Certificate III or IV (or higher) in Disability, Community Services, or a related field

- Minimum 1 year of disability support experience

- Experience supporting high-needs participants and behaviours of concern

- Valid Australian work rights

- NDIS Worker Check and Working With Children Check

- Current Australian driver's licence

- Your own insured vehicle (comprehensive insurance required)

- Solid written and verbal communication skills

- Person-centred and genuinely caring

- Adaptable, organised, and calm under pressure
