Eastern Health is seeking an experienced Registered Nurse for Chronic Rehabilitation in Melbourne's eastern suburbs. The role involves coordinating care for clients across hospital and community settings within a multidisciplinary team, delivering comprehensive assessments, care planning and discharge planning.

Ideal candidates will have a Bachelor of Nursing, NMBA registration via AHPRA, at least five years of clinical experience, and a robust focus on patient-centred care and chronic disease
