The Royal Melbourne Hospital’s Older Person Care Hub (OPCH) seeks a Clinical Nurse Consultant (CNC) Coordinator to lead triage, care coordination and follow-up for older adults at home.

In this part-time fixed-term role, you will work with geriatricians and allied health staff to develop individualized care plans, ensure assessments within 24 hours and support secure transitions, expanding services to seven days by 2027.
