Integrated Care 24 (IC24) is recruiting an Advanced Clinical Practitioner to join our Home Visiting Service across Sussex. You will work autonomously in patients' homes, using advanced assessment, examination, diagnostic and prescribing skills to deliver secure, patient-centred urgent care.

You will care for the full General Practice population, including children, older people and end-of-life patients, with clinical governance support and CPD.
