Hunter Current England Local Health District is seeking an experienced Registered Nurse to join the Hospital in the Home (HiTH) team in Taree Community Health. The role delivers hospital-level care in patients' homes, coordinating with HiTH medical officers, GPs, and acute teams to support safe transitions from hospital to home.

Flexibility to plan your day, workload allocation, and clinical autonomy are key. A seven-day rotating roster and travel to patients' homes are required.
