Barwon Health is seeking a Clinical Pharmacist Grade 1 to provide clinical and operational pharmacy support at the McKellar Centre, with a focus on sub-acute services such as rehabilitation, palliative care and geriatric pharmacy. The role aims to support safe, appropriate and effective medication use and patient flow across hospital and community settings.

You will work with multidisciplinary teams to deliver medication reconciliation, medicines management, patient counselling and discharge
