Mounties Care Home is seeking a dedicated Care Partner to join our Hills District team on a full time basis. You will coordinate and deliver in-home care and services to help clients stay independent at home and in the community.

As part of a supportive team, you will develop care plans, manage budgets, assist clients with My Aged Care, and ensure continuity of high-quality care. Strong coordination and communication are essential.
