Proactive Health in Toowoomba is seeking a Social Worker to join Knight Healthcare. You’ll conduct psychosocial assessments, provide counselling, and coordinate services for DVA, NDIS, aged care and private clients within a multidisciplinary team.

You’ll work with clients in the community, develop care plans, advocate for housing and financial supports, and document in line with AASW standards within a national network of 350+ locations. Recent grads welcome.
