Mersey Care NHS Foundation Trust is seeking an experienced Liaison & Diversion Support Practitioner to join the Reconnect team. This role focuses on supporting people through the criminal justice pathway, advising stakeholders and assisting with transition to community services.
You will contribute to the 24/7 Urgent Care Model, coordinating with mental health teams, probation and other agencies to promote secure, timely care and reduce unnecessary emergency service use.
