Bolton Clarke is seeking a Care Partner to lead care planning and coordination for older people living at home. You will work with the clinical team to ensure protected, independent living and compliant care across Support at Home classifications.

The role focuses on developing individualised plans, liaising with GPs and therapists, documenting care, and guiding participants and carers in making informed service decisions. A not-for-profit provider with strong clinical support.
