Equity Works Assoc. Inc. on the Sunshine Coast is seeking compassionate Lifestyle Facilitators to support people with disability in home settings, including short-term accommodation and SIL roles.

You will deliver person-centred active support, assist with daily routines and promote independence, while following care plans and maintaining explicit documentation. Onboarding, supervision and a strong community focus are provided.
