Harrow East Primary Care Network (PCN) is seeking an Advanced Frailty Practitioner to provide autonomous clinical care to frail patients with complex needs across the PCN. The role requires advanced assessment, independent prescribing and solid care coordination within a multidisciplinary team.
You will work with GPs, community teams and social care to identify, review and manage frailty cases, develop personalised care plans and support early intervention to prevent hospital admissions.
