BlueCare is seeking a Clinical Care Partner to coordinate complex clinical and care needs for clients in Bowen, Proserpine and surrounding areas. You will assess, plan and oversee services to deliver person-centred, high-quality care.

You will develop and review complex care plans with the client and multidisciplinary team, manage budgets and liaise with clinical teams to respond to changing needs. A driver’s licence and access to a roadworthy vehicle are essential.
