Hunter New England Local Health District is seeking a Registered Nurse to support older people transitioning from hospital to home across Taree, Forster or Gloucester. The role is permanent part time, 20 hours per week, with a salary of $41.22–$57.88 per hour plus 12% superannuation and benefits.

You will assess patients, develop plans, and coordinate care with families and a multidisciplinary team to promote independence and safe home-based recovery.
