CareFirst Community is seeking a Community Services Partner to coordinate protected, person-centred supports for NDIS participants and aged care clients. You will plan, monitor and review services with families and professionals, upholding rights, dignity and informed choice.
You will liaise with families, coordinate referrals, manage risks and contribute to a collaborative, compliant team. A current driver license and working with children/card clearances are required.
