NSW Health is seeking a Care Coordinator in Palliative Care, with AHPRA registration as a Registered Nurse and post-registration qualifications in Palliative Care. A minimum of three years' clinical experience in palliative care is required.
The role requires leadership within a multidisciplinary team, and the ability to apply evidence-based practice, advocate for person-centred care, and drive quality improvements. Willingness to travel within the Inland Network is expected.
