Current Life Shelter in New York is seeking a dedicated Case Manager to organize comprehensive service planning and assessment for residents. You will coordinate with housing, mental health, and healthcare providers to ensure integrated care and smooth transitions.

The role emphasizes crisis intervention, ongoing advocacy, and building individualized service plans with discharge planning goals. Onsite work schedule Sunday–Thursday, 9:00am–5:30pm, with comprehensive perks.
