Western Health, Melbourne, is seeking an experienced Registered Nurse to join the Western Health at Home Connect team as Intake and Triage Nurse. You will lead clinical triage, nursing assessments, care coordination and support via telephone and telehealth.

You will provide intake, triage and coordination for Western Health at Home services, delivering expert nursing advice to consumers, families and professionals, and coordinating referrals to ensure timely access to care.
