Western NSW Local Health District is seeking an experienced Clinician to lead the implementation of the Older People’s Aftercare Service pilot and support the district’s suicide prevention pathway. You will coordinate aftercare support, facilitate care planning and service navigation, and provide consultation to clinicians and service partners within a multidisciplinary team.
This hybrid role combines virtual and face-to-face delivery across Western NSW, with hours from 8:30am to 5:00pm, Monday
